initial consultation

During the initial consultation, Dr. Huynh will perform a thorough evaluation and explain your diagnosis and treatment options. He will also recommend an anesthetic course tailored to your needs to ensure your treatment is as pleasant as possible.

To facilitate your visit, please bring the following to the consultation:

• Referral slip from your doctor (or ask your doctor to refer online)
• List of any medications you are currently taking and their dosages
• Form of identification, such as a driver's license
• Insurance card, so we may contact your insurance company and file any claims on your behalf

Our financial coordinator will discuss payment options with you after your treatment consultation with Dr. Huynh. A variety of financial arrangements are available, including financing plans and insurance coverage. Payment for the initial consultation and any X-rays is due on the day of the visit.

NOTE: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.
